predetermination/special authorization

A predetermination or special authorization letter may be sent to your insurance provider to identify fertility coverage. Discuss the details of what should go into the letter with your healthcare provider.

The letter should include:

  • Insurance company name and address
  • Policy holder’s name and address
  • Group policy name and number
  • Policy holder’s identification number

Inform them that you want a predetermination of benefits for fertility treatment. Include as many specific details as possible, based on the plan you and your healthcare provider have chosen.
